Wheelchair Express Van passes with oncoming traffic, over double-yellow, and too close to me

Wheelchair Express Van #87 (license plate 11259G2) passed with oncoming traffic and over the double-yellow.
I was wearing high-vis, red flashing lights (2) on the back of the bike and helmet, as well as one front strobe, and was using my bike horn (Hornit dB140) before, during, and after the close call. My front and back Cyliq footage was sent to Wheelchair express with a sincere invitation for the family owners, or any employee, to ride a bicycle with me; I'd like them to see what I experience first-hand while bike-riding.
